Biological Effects Of Laser Therapy

1. Accelerated Tissue Repair And Cell Growth
Accelerate cellular reproduction and growth. No other physical therapy modality can penetrate the bony patella and deliver healing energy to the articular surface between the underside of the patella and the femur. Cells of cartilage, bone, tendons, ligaments and muscles are repaired faster as a result of exposure to laser light.

2. Reduced Fibrous Tissue Formation
Laser therapy reduces the formation of scar tissue following tissue damage and acute and chronic inflammatory processes. This point is paramount because fibrous (scar) tissue is less elastic, has poorer circulation, is more pain sensitive, weaker, and much more prone to re-injury and frequent exacerbation.

3. Anti-Inflammation
Laser light therapy has an anti-inflammatory effect, as it causes vasodilation and activation of the lymphatic drainage system. As a result, there is a reduction in swelling caused by biomechanical stress, trauma, overuse, or systemic conditions.

4. Analgesia
Laser therapy has a beneficial effect on pain through the suppression of nerve signal transmission over unmyelinated c-fibers that transmit pain to the brain. This means that a greater amount of stimuli is required to create an action potential within the nerve to signal pain. Another pain blocking mechanism involves the production of high levels of pain killing chemicals such as endorphins and enkephalins from the brain and adrenal gland.

5. Improved Vascular Activity
Laser light will significantly increase the formation of new capillaries (angiogenesis) in damaged tissue that will speed up the healing process. Additionally, it has been noted in the literature that microcirculation increases secondary to vasodilation during laser treatment.

Cold Laser vs Hot Therapeutic Laser

The majority of therapeutic laser equipments used are commonly known as “cold lasers”. These lasers have very low power and for that reason do not generate any heat on the skin. Treatment with these lasers is known as “Low Level Laser Therapy” (LLLT).

Class 4 therapy lasers are “hot lasers”. These lasers are much more powerful than cold lasers normally more than 100x more powerful. Therapy with these lasers feels warm and soothing because of the higher energy. This therapy is known as “High Intensity Laser Therapy” (HILT).

Hot lasers due to the significantly higher power, tend to penetrate deeper and create a significantly higher photon density at the target tissue. This significantly increases photon uptake by the compromised cells and rapidly increases ATP production in the Mitochondria. Cold lasers, due to significantly lower power, take much longer to accomplish photon density. The depth of penetration is also determined by the wavelength of the light. A 30 Watt hot laser will treat an arthritic knee to the point of pain relief, in about 5 minutes. A 150 milliwatt cold laser would take over 16 hours to deliver the same dose.


Class IV laser therapy is effective for dealing with a wide range of issues in an effective manner.

For example, with Plantar Fasciitis, which is an inflammation of the band of tissue (the plantar fascia) that extends from the heel to the toes, you can quickly achieve remarkable results starting from the first treatment. In this condition, the fascia first becomes irritated and then inflamed, resulting in heel pain. Heel pain may also be due to other causes, such as a stress fracture, tendonitis, arthritis, nerve irritation, or a cyst. Regardless of the cause, the laser therapy quickly reduces both pain and inflammation and sets in motion a cascade of healing responses. A treatment of such a condition may proceed as follows:


    • Continuous wave laser therapy, typically 15-30 watts.

    • Typically 3000-4000 joules, delivered through 10-15 min treatments.

    • Defocused beam (approx. 4-5 cm) is applied to specific points, targeting the inflammation (and pain) source.

    • The laser is set to high power laser therapy program, that delivers maximum continuous laser power for several minutes. Intermediate area massage is needed to increase blood flow and allow excess heat to dissipate.

  • Treatment is repeated 4-6 times, 1-2 times per week. 

Laser therapy is the application of red and or infra-red electro-magnetic energy, which enters the body as photons. Photons are small packets of light energy which are delivered through laser beams, which stay fairly narrow and do not diverge like a regular light bulb or LED. The amount and depth of penetration from this photonic energy is significant, and dependent upon a number of different factors related to the patient and the power of the laser therapy machine. The most therapeutically effective lasers are powerful Class 4 multi-wavelength lasers that can deliver 90-100% duty cycle at continuous wave (CW). Research shows that tissues targeted by High Power Laser Therapy are stimulated to increase production of a cellular enzyme called Cytochrome C Oxidase, which is critical to the production of ATP. ATP is the primary  fuel for all living cells. With increased ATP production at the target tissue, and thus greater cellular energy, a host of biological responses is facilitated. Pain mediation is achieved, inflammation is reduced, cellular metabolism is increased , vascular activity is improvedscar tissue is reduced or dissolved, and accelerated healing is achieved.
